Waiver / Consent Form

I am above 18 years of age.

I will ensure that all members of this registration follow all instructions provided by MYPI staff.

I understand that I am responsible for the health and safety of all members of this registration, especially those under the age of 18.

I hereby execute this Release in favor of the Muslim Youth for Positive Impact (MYPI)

• By signing this Release, the Releasor acknowledges, understands and agrees that, in consideration of MYPI allowing the Releasor to participate in the Released Activities, the Releasor waives any and all claims against MYPI, whether based on contract, negligence or otherwise, for damages of any nature or kind the Releasor may suffer as a result of his or her participation in the Released Activities. Releasor expressly releases, and agrees to forego any legal action against, MYPI and its officers, employees, agents and assigns as to any claims, costs or expenses the Releasor may have that arise from or out of the Released Activities.
• Nothing in this Release shall be construed to waive, limit or otherwise modify any governmental immunity that may be available under the Colorado Governmental Immunity Act, C.R.S. §§ 24-10-101, et seq., to MYPI and its officials, employees, agents or other persons acting on the MYPI’s behalf.
• Should any provision of this Release be held invalid, illegal or unenforceable, it shall not affect or impair the validity, legality or enforceability of any other provision of this Release.

PARENTAL CONSENT AND RELEASE FORM

• My child(ren), named above, has (have) permission to participate in this event. I understand that this activity may cause injury to the participants.
• ACKNOWLEDGEMENT OF PERSONAL LIABILITY AND WAIVER: I understand that this event may expose my child(ren) to some risks and I assume any such risk that may arise there from. I accept full responsibility for all medical expenses for any injuries that might occur to my child(ren) by reason of his/her/their participation. By signing this form, however, I hereby release MYPI from and against any and all claims, demands, actions, complaints, suits or other forms of liability that any of them may sustain arising out of any unfortunate events.
• I also confirm that I am the parent or legal guardian of the child(ren).
